Pharmaceutical Care (PC) Competency

Pharmaceutical care competency are pharmaceutical knowledge, attitudes, skills and behaviours a pharmacist acquires, accumulates and develops through education, training and work experience. With the above attributes, a pharmacist can carry out duties accurately with all confidence. This also includes making correct judgement and interacting appropriately with patients and other healthcare professionals.

Competency is characterized by a strong knowledge base, good problem-solving and decision-making abilities and ability to apply knowledge and experience to diverse patient care situations.

Competencies

1. Design and evaluate therapeutic regimens to optimize drug use.

2. Design and implement strategies that influence prescriber and patient behavior to achieve optimal outcomes.

3. Apply pharmacokinetic and pharmacologic parameters to prevent, manage, or resolve drug-related problems.

4. Acquire and apply patient-specific clinical and laboratory data to support therapeutic decisions.

5. Teach appropriate drug use, the medication use process, self care, and principles of preventative health to people or groups from diverse backgrounds and cultures and other health professionals.

6. Document and evaluate professional services and interventions.

7. Understand and participate in ADR reporting, management of quality control, and drug security systems.

8. Document, evaluate, and manage adverse drug reactions and medication errors.

9. Apply pharmacoeconomic and formulary management principles to achieve cost effective outcomes for patients of institutions or health-systems.

10. Participate in the management of drug distribution systems, human resources, and information systems.

11. Conduct structured and rigorous evaluations of the health care literature.

12. Provide drug information to diverse audiences.

13. Analyze and interpret financial documents to support management decisions.

14. Provide first care, triage patients as appropriate, and collaborate with other health care professionals to share responsibility for continuity of care and effective patient outcomes.

15. Identify gaps in health care delivery or the medication use process and design, implement, and evaluate strategies to resolve (project).

1. Knowledge in managing therapy and communication with patients and other caregivers.

2. Skills which include pharmacotherapy, physical assessment, drug information, communication, critical thinking and problem solving skills.
