Form K is a book. It is sold only to community pharmacies. It is a book that is used to document sales and prescription of control drugs (CD) in Nigeria.

Control drugs are drugs that require a medical prescription before a pharmacist can dispense. They are also called poison. This is because little dose adjustment, prolonged use or abuse can have damaging effects to the user.

When a patient comes with a prescription for a CD, the pharmacist interprets the result. After dispensing, the prescription is kept in the poison book and the details of the patients are documented in the form K.

Name of the patient

Signature of the pharmacist
