What Is Form K And Poison Book In Pharmacy
Form K is a document sold by the pharmacists council of Nigeria (PCN). PCN is responsible for the regulation of training and practice of the pharmacy profession.
What Is Form K
Form K is a book. It is sold only to community pharmacies. It is a book that is used to document sales and prescription of control drugs (CD) in Nigeria.

Control drugs are drugs that require a medical prescription before a pharmacist can dispense. They are also called poison. This is because little dose adjustment, prolonged use or abuse can have damaging effects to the user.
Form K |
When a patient comes with a prescription for a CD, the pharmacist interprets the result. After dispensing, the prescription is kept in the poison book and the details of the patients are documented in the form K.
The following data are documented;
Date of dispensing
Name of the patient
Name of prescriber
Name of the drug and quantity
Signature of the patient
Signature of the pharmacist

What Is Poison Book
Poison book is a book for dispensing CDs. It is the same thing with form K. Form K is called disposal of a poison book.
