Register For NYSC Orientation Camp Self Reporting/Health Declaration Form
When covid-19 was discovered in Nigeria, the management of the national youth service corps (NYSC) suspended orientation camp programmes nationwide. This was soon followed by weekly community development service (CDS) and monthly clearance. Little did we know that it would last up to nine months. NYSC management recently announced the reopening of NYSC orientation camps nationwide including the federal capital territory (FCT), Abuja. Because there is still covid-19 in Nigeria, the management of NYSC requires all orientation camps to follow the Nigeria center for disease control (NCDC) guide on gathering of people. However, it also requires prospective corp members (PCM) to present a self reporting form/Health declaration form. This form may have to be filled by both officials and PCM before coming to camp.

Self reporting form/health declaration form was a guide prescribed by the world health organisation (WHO) for travellers and those who wish to come together. It has been adopted by our Nigeria civil aviation authority (NCAA) in march, 2020. It means that those who wish to come to the country must follow some protocol. It requires travelers to visit the NCAA website link to NCDC to fill a form. The information needed included which country you are coming from and if you have lived in that place for more than 21 days. Others are how you are feeling and which part of the country you wish to stay. The completed form is downloaded and sent to NCAA.

On arrival, the hard copy is given to the personnel of port health services for evaluation. This helps in accessing those at high risk of developing covid-19.
PCM Going To Camp
There are so many other processes that will be discussed later. But since NYSC is different from travel, the process may be different. However, the question will be very similar. This process is cheaper for the government than mass testing which may be at the cost of the government. But for PCM coming from other countries, they must pass through the port and follow the process.
This is the link. The form must be filled and downloaded. The information may include questions where you have been in the last 21 days and if you have had any close contact with a covid-19 patient. On arrival at the gate of the orientation camp, the downloaded form will be submitted to a health personnel for evaluation. A thermal body check may be carried out and questions about health status before PCM will be allowed into the camp.
Foreign Train PCM
NCAA requires that those coming into the country must present a negative covid-19 Polymerase chain reaction (PCR) test maximum of 96 hours before boarding any airline. However, 72 hours before boarding any airline is best. This is when the test is taken and not when the result is given. Then a second test to be conducted in Nigeria must be paid for in the country to which you are coming from. To make the process in Nigeria easy, payment should be made before boarding any airline by visiting the NCAA website.
There are three COVID testing protocols for inbound passengers:
Step 1: Complete the health declaration and contact form via Nigeria’s online International Travel Portal (nitp.ncdc.gov.ng).
Step 2: Upload negative PCR test results (administered within 96 hours of departure) to the online portal, and present a hard copy at check-in. For certain countries, the test must be from specified labs.
Step 3: Register and pay via the online portal for a PCR retest in Nigeria to occur seven days after arrival

Any passenger unable to complete payment will need to present a negative PCR result. They will still need to pay for a repeat covid-19 PCR test on arrival. Passengers without QR code may face longer waiting time. All passengers must self isolate preferably in the state of port for eight days. If the second covid-19 PCR test result is negative, they can go about their normal business of proceeding to NYSC orientation camp. But if positive, can self isolate for 14 days or go for treatment according to NCDC guidelines.
