The national youth service corps (NYSC) provide an identity card for all corp members (CM). CM are expected to be with it at all times. It is one of the required documents needed for CM to be given their discharge certificate.
However, due to carelessness on the parts of most CM, it is not abnormal to hear about CM losing it. And because most thieves target lady handbags and guys wallet, many CM loss their NYSC identity card through this means also. So let us see what can be done about this.

Lost Or Missing NYSC Identity Card

NYSC issue two identity card to all CM. The first is the state code tag laminated and given to CM at NYSC orientation camp. The other is the NYSC plastic identity card issued days to the end of the orientation camp. The laminated state code tag is returned to NYSC to get the plastic identity card.

How To Replace A Lost Laminated State Code Tag

At the NYSC orientation camp, many CM loss their laminated state code tag. Failure to get another one can cause you some headache such as failure to get camp food. You may also run into trouble when you want to get your plastic identity card without your state code tag. To avoid stories that touch, go to registration center.
Explain to them what happened. They will ask you for your state code. They will use permanent bblack marker to write your state code on a white paper with an identity card size. At the bottom, the official in charge will affix a signature. All this at no cost to you. You can use this anywhere within the camp. You can also use it to collect your plastic identity card.

How To Replace A Lost Plastic Identity Card

The NYSC plastic identity card is very valuable. If you lose it, the process of replacing it is much more stressful. But is important for passing out parade.

Offline

Go to the nearest police station  and report the case. They will give you a police report.  Take the police report to a magistrate court for a sworn affidavit. Make copies of the police report and the sworn affidavit.
Write an application letter to the state coordinator explaining what happened and what you want. Attach the police report and affidavit with the letter. Give the documents to your Local government inspector (LGI). Pay the official fee.
Your LGI will cco tact you when your card is ready. Do not wait until the close to passing out parade before applying.

Online

There is now a modern way of applying for replacement of the plastic identity card. Immediately after passing out of camp, a new link is introduced in the dashboard. The link is label 'replacement of ID card.' Click on it. It will take you to a new page where you can see your name, call up number and state code. A dialog empty box open for you to explain what happened. Type in your request and click the send request.
This will open remita page where you will be required to make payment. You can pay online using your bank card or at any bank branch near you with your RRR. complete the payment.
After payment, you will be required to upload a police report and the sworn affidavit. A new plastic identity card will be sent to your local government area where you LGI is.
