Application For Entry In The Index Of Students For Pharmacy Programme In Nigeria

The Pharmacists Council of Nigeria (P.C.N) is a Federal Government Agency that is charged with the responsibility of regulating and controlling the practice and education, training of pharmacists and pharmacy technician in all aspects and ramifications. They determine the standard of knowledge and skills by pharmacy students. This may be the reason for the introduction of PCN student register, which is called pharmacy student indexing.
Student Indexing is a mandatory procedure for the registration of students who intend to pursue a Pharmacy program by assigning them unique identifiers (index number). Indexing will provide a link between the learners, training institutions, and the PCN.
The pharmacy student register (index) which was introduced this year is to be filled by undergraduate pharmacy students and must be submitted to PCN and may be used by PCN to checkmate the activities of pharmacy schools. There may be other reasons. The information provided can be very vital for so many things.

Demography

The information range from the name of the student to marital status, etc. This information must correspond with other documents to be submitted.

School

The name of the school as well as the address.

Students' Details

This is the most important information I guess. It can give precise details of students admitted every year by any one school. This can help check mate schools that offer admission beyond the number approved by PCN. Information needed is the date of admission and matriculation number.

Past Educational Record

PCN has given a benchmark for minimum academic requirements for qualification to study pharmacy. This information will provide them with such details and see those who meet it. They include;

  1. Primary School Leaving Certificate and date
  2. Junior School Certificate and date
  3. Senior School Certificate Examination (S.S.C.E) (O’ Level) and date
  4. A’ level certificate and date
  5. and any other past qualifications

Photocopies

The form must be submitted with attached photocopies of the following documents;

  1. University/Joint Admission and Matriculation Admission (J.A.M.B) letter
  2. Birth certificate or statutory declaration of age
  3. O’ level result or its equivalent
  4. One recent passport

For the time been, all students irrespective of level must fill this form and submitted to the dean of the faculty. This will be forwarded to the registrar of PCN. However, it seems this process will continue as new undergraduate pharmacy students must fill and submit this form as required by PCN. It should be submitted two weeks after gaining admission.

There is a place for the student to sign signifying that all provided information is correct and accurate. The signature of the dean follows this.

For now, the process is manual. hope PCN will upgrade this process so that students can submit it individually online.

If pharmacy students in Nigerian university are required to do this, there is a high probability that pharmacy technicians’ students will also be required to do it. They should check with their local school authority.
