ADay Kit Composition, Dose, Use, Side Effect

ADay kit is a combination therapy. It contains an antibiotic, antifungal and an antiprotozoal agent. It is mostly prescribed as an emergency prevention therapy against sexualy transmitted infections (STI). It is manufactured in India by The Madras Pharmaceuticals and marketed in Nigeria by Clarion Medicals.

Composition Of ADay Kit

Each kit contains:

(A) Fluconazole Tablets USP (1 tablet)

Each uncoated tablet contains:

Fluconazole USP...150 mg

(B) Azithromycin Tablets USP (1 tablet)

Each film-coated tablet contains:

Azithromycin Dihydrate USP

equivalent to anhydrous Azithromycin...1 gm

(C) Secnidazole Tablets (2 tablets)

Each film-coated tablet contains:

Secnidazole…1 gm

Indication Of ADay Kit

Secnidazole Tablets: Secnidazole is primarily indicated in conditions like Amoebiasis, Giardiasis, Trichomoniasis, Urethritis, and Vaginitis.

Azithromycin Tablet: Azithromycin is effective against susceptible bacteria causing otitis media (infection of the middle ear), tonsillitis, laryngitis, bronchitis, pneumonia, and sinusitis. It also is effective against several sexually transmitted infectious diseases such as nongonococcal urethritis and cervicitis.

Fluconazole Tablet: For the treatment of fungal infections.

Mode Of Action

Secnidazole Tablets: Secnidazole is active against E histolytica, G lamblia, T vaginalis, Clostridium spp, B fragilis, Gardnerella spp. The drug enters the microorganisms by diffusion and is reduced intracellularly by low oxidation-reduction potential ferredoxin which then results in DNA damage.

Pharmacokinetics: Renal Excretion accounts for 50% and plasma half life is 20 hr.

Azithromycin Tablet: Oral absorption of Azithromycin is found to be 34% +19. Volume of distribution is found to be 31.1 1/kg and plasma protein binding is 51%. Renal Excretion accounts for <10% and plasma half life is 68%.

Fluconazole Tablet:

Absorption: Well absorbed after oral admin; peak plasma concentrations after 1-2 hr (oral), may be increased with multiple dosing.

Distribution: Widely distributed. Protein-binding: 12%.

Excretion: Via urine (80% as unchanged, 11% as metabolites); 30 hr (elimination half-life); increased in renal impairment. Removed by dialysis.

Dosage And Administration Of A Day Kit

A) Fluconazole 150 mg - Take 1 tablet as a single dose with Secnidazole or alone after breakfast.

B) Azithromycin 1g - Take 1 tablet as a single dose at least 1 hour before or 2 hours after meal (lunch).

C) Secnidazole 1g - Take 2 tablets as a single dose with food (dinner) to avoid gastrointestinal side effects.

Contraindication

Secnidazole Tablets: Pregnancy, Lactation, Hypersensitivity

Azithromycin Tablet: Azithromycin is contraindicated in conditions like Hepatic impairment, Hypersensitivity.

Fluconazole Tablet: Hypersensitivity.

Side-effects Of ADay Kit

Secnidazole Tablets: Nausea, epigastric pain, glossitis, fatigue, headache, loss of appetite, diarrhea stomatitis, urticaria rash and leucopenia.

Azithromycin Tablet: Mild to moderate nausea, vomiting, abdominal pain, dyspepsia, flatulence, diarrhea, cramping, angioedema, cholestatic jaundice; dizziness, headache, vertigo, somnolence; transient elevations of liver enzyme values.

Fluconazole Tablet: Common side effects include headache, nausea, and abdominal pain. Other side effects included diarrhea, dyspepsia, dizziness, rash, and taste disturbance. Rarely, serious allergic reactions, including angioedema (swelling of tissues), may also occur. Liver dysfunction (jaundice, hepatitis) has also been associated with fluconazole.

Warnings / Precautions

Secnidazole Tablets: Alcoholic drinks and alcohol containing medicines should be avoided during secnidazole treatment. Do not administer to subjects with a history of blood dyscrasia.

Azithromycin Tablet: Azithromycin should be used with caution in patients with illness or any allergy, especially allergies to drugs, liver disease, jaundice, history of colitis or stomach problems or kidney disease. This drug should be used only if clearly needed during pregnancy or lactation.

Fluconazole Tablet: Fluconazole should be used with caution in patients with liver disease, kidney disease or of any drug allergy. This medication should be used only when clearly needed during pregnancy. Avoid fluconazole during lactation.

Pregnancy And Lactation

Secnidazole Tablets: Secnidazole may be prescribed in pregnancy after the first trimester. As with other similar drugs, secnidazole should not be administered during the first trimester of pregnancy or during lactation because secnidazole is found in placenta and breast milk.

Azithromycin Tablet: Caution when used during pregnancy and lactation.

Category B: Either animal-reproduction studies have not demonstrated a fetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1st trimester (and there is no evidence of a risk in later trimesters).

Fluconazole Tablet: There are no adequate studies in pregnant women. There are reports of harmful effects to the fetus when high dose fluconazole was administered to pregnant women for a few months.

Use of fluconazole during pregnancy is not usually recommended.

Nursing Mothers: Fluconazole is secreted in breast milk. Use of fluconazole by nursing mothers is not recommended.

Interaction

Secnidazole Tablets: Potentiate anticoagulant effect of warfarin. Concurrent disulfiram may cause ant- abuse like effects. Cimetidine prolongs halflife.

Azithromycin Tablet: Azithromycin is known to interact with other drugs like Aluminum Hydroxide and Oxide, Astemizole, Cyclosporin A, Digoxin, Terfenadine, Theophylline, Triazolam. These interactions are sometimes beneficial and sometimes may pose threats to life. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.

Fluconazole Tablet: Fluconazole is known to interact with other drugs like Alfentanil (HCI), Aliskiren, Alprazolam, Amitriptyline (HCI), Artesunate, Astemizole, Bosentan, Celecoxib, Chlorpropamide, Cisapride, Cyclosporin A, Fosphenytoin, Hydrochlorothiazide, Loratadine, Losartan (K), Nortriptyline (HCI), Phenytoin (Na), Quetiapine, Repaglinide, Rifabutin, Rifampicin, Ritonavir, Saccharomyces, Sulfapyridine, Sulfamethoxazole, Terfenadine, Theophylline, Torasemide, Warfarin (Na), Zidovudine.

These interactions are sometimes beneficial and sometimes may pose threats to life. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
