Klovinal Vaginal Tablet Composition, Dosage, Use, Side Effects

Klovinal vaginal tablet also called pessary is a drug containing metronidazole, clotrimazole and lactobacillus spores. It is used for the treatment of vaginal infection. Metronidazole and Clotrimazole destroy both the good and bad infection of the vaginal.

The good bacteria in the vagina protect it from some form of infection and conditions. To restore normal vaginal flora, the lactobacillus spores supply fresh good bacteria back to the vagina.

Klovinal vaginal tablet comes in 6 tablets in a pack. The tablet is white to off white bullet shaped pessaries. It is made by bliss GVS pharma ltd in India and marketed by seagreen pharmaceuticals ltd in Nigeria.

Composition Of Klovinal Vaginal Tablet

Each Pessary contains:

Metronidazole BP 500 mg

Clotrimazole BP 100mg

Lactobacillus Spores 150 millions

Pharmacodynamics Of Klovinal

Clotrimazole is a broad spectrum antifungal agent available for topical or intravaginal use. It is active against yeast and dermatophytes and also some Gram +ve bacteria, especially Staphylococcal and against Trichomonas. Clotrimazole acts by altering the permeability of fungal cell walls by interfering with cell wall lipids.

Metronidazole is active against several protozoa including Trichomonas vaginalis. It is bactericidal. Metronidazole is used in the treatment of susceptible protozoal infections and in the treatment and prophylaxis of anaerobic infections. Specific bacterial infections treated with Metronidazole include bacterial vaginosis (nonspecific vaginitis).

The vagina is a dynamic ecosystem that normally contains approximately 10⁹ bacterial colony forming units per gram of vaginal fluid. The normal bacterial flora is dominated by Lactobacilli. Lactic acids and other organic acids are metabolized from glycogen by the lactobacilli, maintaining the vaginal pH between 3.8 and 4.2. The acidic environment inhibits the overgrowth of bacteria and other organisms with pathogenic potential.

Because of vaginal infections like bacterial vaginosis, trichomoniasis and vulvovaginal candidiasis, the Lactobacilli flora goes down and vaginal pH increases to more than 4.5. Because of anaerobic bacteria, musty or fishy odour starts. Lactobacillus is in spore form. It is used to recolonize the vaginal flora and to maintain the vaginal pH between 3.8 to 4.2

Pharmacokinetics Of Klovinal

When applied topically, clotrimazole penetrates the epidermis but there is little if any systemic absorption has been reported following vaginal administration. Absorption of Metronidazole from vaginal pessaries has been reported to be poor. Lactobacillus sporogenes recolonize the vaginal flora.

Indications And Use Of Klovinal

Klovinal pessary is indicated for vaginal infections like trichomoniasis, bacterial vaginosis and vulvovaginal candidiasis.

Dosage Of Klovinal Tablet

One klovinal pessary should be inserted into the vagina as deeply as possible once daily. It should be used for 6 days, preferably on retiring to bed at night.

Can Klovinal Tablet Be Use During Period

Yes if you have already started the treatment. Do not interrupt the course of treatment. Continue until the prescribed course is finished.

If you are just prescribed and your period started, you can start if the infection is severe. If not, wait until your period is over. Using klovinal during period is usually messy.

Contraindications

Klovinal pessary is contraindicated in patients known to be hypersensitive to topical Imidazoles.

Warning And Precautions

If irritation or sensitivity develops with the use of klovinal pessary, treatment should be discontinued and appropriate therapy instituted. If there is lack of response to klovinal pessary treatment, microbiological studies should be done to confirm the diagnosis and rule out other pathogens before instituting another course of anti mucolytic therapy.

Pregnancy And lactation

Klovinal pessary should only be given to a pregnant or lactating motor if benefits outweigh the possible risk to the fetus.

Interaction

Antibiotics with activity against lactobacillus should be avoided. This includes home remedies like cedar vinegar, baking soda, etc.

Side Effects And Adverse Reactions Of Klovinal

Few patients have reported local pruritus and burning sensation after vaginal administration.

How To Use Klovinal

1 Wash your hands

2. Do not open pessary if it seems soft, hold the foil wrapper under cold water or place it in refrigerator for few minutes to harden it before removing the wrapper

3. Remove one pessary from the strip and hold it

4. Remove any foil or plastic wrapping stuck to the pessary

5. Sit or lie down with your knees bent and legs apart

6. Gently insert the pessary into the vagina as far as comfortably possible using your fingers

7. Wash your hands again

8. Remain in that lying position for about an hour and do not stand up to use the toilet

How Long Does It Takes For Klovinal To Dissolve And Absorb

It should start to dissolve in about 10-15 minutes. At this stage, it can leak out if you stand up. It takes about an hour from the time of insertion for it to be fully absorbed into the body. It is at this stage you can carry out a normal routine. Do not remove the tablet after insertion. It must remain there and absorb into the body.

Can A Virgin Use Klovinal Tablet

Virgin can use a klovinal tablet. Virgins have a little opening in their vagina where menstrual blood flow passes. Careful insertion of the klovinal pessary into that opening is possible.

Can Klovinal Be Use In Pregnancy

Klovinal is contraindicated in early pregnancy. Clotrimazole is generally considered safe for pregnant women to use. Metronidazole is not used in early pregnancy because of potential adverse effects on the fetus.
