NYSC CDS Project Final Report Sample
Community Development Service (CDS) is one of the four cardinal programmes of the NYSC in which corps members contribute positively to the development of their host communities throughout the period of national service. Personal CDS project can be embarked upon on a proposal to NYSC through the local inspector. Approval must be sought before executing the project.
It is expected of all corp members undergoing any personal CDS project to give bimonthly reports. You can read more about that here. In conclusion, you are expected to submit a final report.

APPLICATION FOR A PERSONAL CDS PROJECT
In respect of the National Youth Service Corps scheme that corps member(s) should render community development services that will meet with the present needs of their community, I Ugbede Emeoyo Gaius with code number DT/15B/0738 wants to embark on the following projects for the development of ozoro which is my place of Primary assignment.
Malaria is a life-threatening disease caused by parasites that are transmitted to people through the bites of infected female Anopheles mosquitoes. It is preventable and curable. In 2019, there were an estimated 229 million cases of malaria worldwide. The estimated number of malaria deaths stood at 409 000 in 2019.
Some population groups are at considerably higher risk of contracting malaria, and developing severe disease, than others. These include infants, children under 5 years of age, pregnant women and patients with HIV/AIDS, as well as non-immune migrants, mobile populations and travellers. National malaria control programmes need to take special measures to protect these population groups from malaria infection, taking into consideration their specific circumstances. Children aged under 5 years are the most vulnerable group affected by malaria; in 2019, they accounted for 67% (274 000) of all malaria deaths worldwide.

My profound gratitude goes to God Almighty for his inspiration and mighty help throughout my service year in Ozoro to bring me to the right people and bring the right people my way to support my work. CHAPTER ONE:
1. Malaria in pregnant women and children under one Awareness
2. Aim: This awareness was aimed at enlightening the unreached and uninformed people of Isoko North of the significance of malaria in pregnant women and children under one.
3. Drive: The drive came as a result of the sudden death of people due to the disease.
4. Report on malaria in pregnant women and children under one awareness, prevention and distribution of insecticide treated net In Ozoro on the 21st of January 2016.
In respect of the National Youth Service Corps scheme that corps member(s) should render community development services that will meet with the present needs of their community, due to the recent and alarming casualty of malaria in pregnant women and children under one, I embarked on a 1(one) day awareness in ozoro in Isoko North Local Government Area on 21st January 2016.
Ugbede Hall: The awareness started by 7:55 AM in Ugbede Hall, ozoro road where a fellow corps member Segi Samuel with code number DT/15A/1663 explained the cause, prevention and remedies of malaria fever to the women. Questions were raised by those in attendance and the questions were answered.
Ozoro General Hospital: We went to Ozoro general hospital for the awareness and the head of department of the hospital immediately called out the women to the hospital hall for the awareness. Miss Ifeoluwa Damilola continued the awareness.
Mode Of Dressing: We were properly dressed with a NYSC T-shirt, Kaki and NYSC cap.
Sponsorship: The transportation cost was taken care of by Mr. James Ogedegbe, the head of department of ozoro general hospital, ozoro, Delta state. The awareness was brought to a close at exactly 9:30 AM.
Approval: The approval for this awareness was sought from the NYSC and approval given. As earlier attached is the letter of approval.
Achievement/ Result: The awareness covered Ozoro general hospital, Ozoro and Ugbede Hall in Ozoro.
CHAPTER TWO
1. Enlightenment campaign on malaria in pregnant women and children under one including distribution of insecticide treated net.
Aim: The enlightenment campaign was aimed at pregnant women and nursing mothers with children under one. This is because they are more vulnerable to casualty of malaria infection in endemic areas like Ozoro with less health care facilities.
Report on the enlightenment campaign on malaria in pregnant women and children under one and distribution of insecticide treated net to the ozoro community.
Ozoro is a town located in Isoko North Local Government Area, Delta state. Upon arrival to the town, I found out that there are many cases of malaria in pregnant women and children under one. Some don't make it out alive. I decided to distribute a “insecticide treated net” to solve the problem of malaria transmission. I met with the councilor of Ward 2, in Isoko North Local Government Area and the chairman and was pleased with the idea and gave his support for the project. He also gave me the necessary support technically which enabled me to raise financial support for the work. The village youths were helpful in the organisation and distribution of the insecticide treated net.
It was officially handed over to the community head on the 18th of August, 2016 in the presence of the Primary Health care workers, some corps members and a call to the NYSC Local Government Inspector/Asst. Local Government Inspector and the Zonal Inspector. Sponsorship: The insecticide treated net was sponsored by Hon. Ovie Oza Israel the Ward 2 Councilor, Isoko North LGA. Partnership: The project was executed in partnership with the Ozoro women development union who were there to ensure accessibility to the most vulnerable groups
Approval: The approval for this project was sought from the NYSC and approval given. As earlier attached is the letter of approval. Achievement: The insecticide-treated net can be used by pregnant women and children under one in the village to protect themselves from mosquito bites.
CHAPTER THREE:
Skills Acquisition And Entrepreneurship Development (SAED)
Aim: To see that the youths in Ozoro learn other skills and be acquainted in other than their nominal farm work and if possible, make a living from the acquired skills.
Report On Youth Empowerment Programme In Ward 2 Isoko North LGA
In my findings and discussion with some of the youths in the village, I discovered that most youths in ward 2 have only Okada riding and cassava farming as their view of tomorrow. This mindset sometimes leads to youths not being able to have an entrepreneurial mindset towards life. Due to this I decided to organize a youth empowerment programme for the youths in ward 2. I organized a skills acquisition on the preparation of liquid soap, Dettol, bleach, body perfume, and Izal for some women in Ozoro in Isoko North Local Government Area on 22nd March 2016. The training started at 9:15 AM in Ugbede Hall and two corps members Nurudeen Yusuf with code number DT/15B/1578 and Alaba Hassan with code number DT/15B/1676 registered the youths in attendance from different communities. The program recorded a total number of one hundred and seven (107) youths from different communities in ward 2. The communities include Ozoro, Owhe-ologbo, uzere, Oleh and igbide. The attendance of the skills acquisition program is attached with this report. The training was handled by two corps members Tagbo Onyeka Daniel with code number DT/15A/2083 and Musa Olufisayo Damilola with code number DT/15A/1663, they taught the youths of the communities the processes involved in the production of liquid soap, Dettol, Bleach and body perfume. The production was done by the youths and thoroughly supervised by the corps members. Questions raised by the youths were answered by the corps members. The skills acquisition program was brought to a closure at exactly 12:23pm. The materials for the production were also presented to some of the youths to empower them for production afterwards.
Sponsors: The cost of the empowerment programme was handled by the aspiring Councillor Ward 2 Isoko North, Delta state of the People’s Party Hon. Ovie Agas Israel and the following partners: Ugbede Hall and Ozoro general hospital, Ozoro, Delta state.
CHAPTER FOUR
SCHOLARSHIP AWARD AND COMPUTER TRAINING
Aim: This training was aimed at seeing that the students of Ozoro get adequate awareness on the use of Computer Based Test (CBT) for Aptitude tests.
Report On The Scholarship Award And Computer Training
In respect of the National Youth Service Corps scheme that corps member(s) should render community development services that will meet with the present needs of their community, I Ugbede Emeoyo Gaius with code number DT/15B/0738 organized a computer training programme for all the youths in Ward 2, isoko North L.G.A which held in Ugbede Hall in ozoro in Isoko North Local Government Area on 9th June 2016. Ozoro is a town located in Delta state. Major means of transportation to the town is by bus and private cars with good roads. Okada is the major means of transportation within the town. The power situation is appalling with little social amenities. Basic amenities are subpar. Education here is growing due to the university recently introduced. The training started at 12:03 PM in Ugbede Hall in Ozoro and two corps members Feyisetan Toluwanimi with code number DT/16A/0919 and Akpodono Onajite Marilyn with code number DT/16A/0741 registered the youths in attendance from different communities. The program recorded a total number of over a hundred and fifty youths but the number of persons in register is one hundred and seven (107) youths from different communities in ward 2. The communities include Ozoro, Oleh, Owhe-ologbo, etc. The attendance of the computer training program and the CBT practice is attached with this report. The training was handled by the ICT CDS group of the Isoko North Local Government Area. The topics handled by the group include:
1. Introduction to Computer and Hardware Maintenance
2. Computer Ethics
3. Importance of ICT
4. Computer Networking
5. Importance of Computer Security
6. Causes and prevention of Malware/Computer Virus
7. Introduction to Programming
8. The Use of E-learning as a modern means of Education
9. A total number of fifteen (15) corps members were in attendance for the training.
Scholarship Award
An on hands training was also done for some people and a computer based test was conducted for the youths and Scholarship worth twenty thousand naira each (N20,000) was awarded by Classic System Infotech to three of the outstanding youths in the CBT training. Gifts were also presented to six (6) outstanding youths of Ward 2 by a representative of the honourable councillor for ward 2 Hon. Ovie Agas Israel. Questions raised by the youths were answered by the corps members. The computer training and CBT test was brought to a close at exactly 2:23 PM. Sponsors
The scholarship was given by Classic System Infotech. The scholarship is a free Data Analysis training for the three students. The hall arrangement and Refreshment was done by the Host hall, Ugbede Hall.
The remaining cost of the computer training and CBT programme was handled by the Councillor Ward 2 Isoko North, Delta State, Hon Ovie Agas Israel and the following partners: Attached herewith are some photograph pictures taken that day.
CHAPTER FIVE
Free Medical Scheme and Distribution Of Insecticide treated net
Aim: This project is aimed at seeing that pregnant women and children under one of Ozoro get adequate information on health issues and treatment to those who have one ailment or the other which includes distribution of insecticide treated net to aid prevention of transmission.
Report Free Medical Check-up And distribution of insecticide treated net IN Obunkeko Ward 2 Ondo West LGA And Request For A Recommendation Letter
In respect of the National Youth Service Corps scheme that corps member(s) should render community development services that will meet with the present needs of their community, I Ugbede Emeoyo Gaius with code number DT/15B/0738 organized a free medical check-up, treatment and distribution of insecticide treated nets to the villagers in ozoro in ward 2 in Isoko North Local Government Area on 18th August 2016. The medical check-up started at 12:23pm in the Primary health Centre, ozoro. The villagers went through four series of tests: Body Mass Index Test (BMI), Malaria Rapid Diagnostic Test (RDT), RVST (HIV) Test and Blood Pressure Test.
After the tests, their results were collated and were sent to the medical doctor who gave them medical counselling and prescription of drugs they will need for those having ailment. The pharmacy session saw the delivering of drugs as prescribed by the Doctor and distribution of insecticide treated net.

The children were also not left out as advice was given to them on health matters
The free medical scheme was brought to a closure at exactly 4:07 PM.

CHAPTER SIX
Active Participation in medical CDS group: I actively participated in my Community Development Service Group, Medical group in proffering solutions to challenges and also did special duties assigned to me by the group leader. I was also elected the Secretary of the group.
Active participation in medical CDS group sensitization to hospitals and clinics in Delta State
Firstly, I spoke on malaria in Ozoro polytechnic during one of our sensitizations to educate the girls on how to prevent malaria in order to prevent infection. Secondly, I also participated in the visit to Thy Will Comprehensive College Ayeyemi, Oka for the groups’ project and I taught the students on malaria and it's dangers. I also acted as a member of the sensitization committee and project committee for the group. I sincerely appreciate my Community Development Service group leader for the opportunity and all members of the group for their co- operation.
CONCLUSION/ RECOMMENDATION
It has been a pleasure serving my country in this capacity and meeting the wonderful people of ozoro. It is indeed an unforgettable experience for which I appreciate General Yakubu Gowon who established it. I want to recommend that the NYSC platform be further broadened and modified to suit the present challenges of Nigeria instead of being scrapped. I also recommend that much more emphasis be placed on community development as a major interest for corps members because through these I have learnt a lot more than what was offered in my place of primary assignment. In addition, I wish to recommend that the NYSC management should make funds available for corps members who wish to carry out certain Community Development Service of which may be financially handicapped in terms of sponsors.
